Monday, May 21st is the last day for physicals to be done here at CCCHS by our physician. Please turn in the 11th grade physical consent form that was recently mailed to Parents/Guardian ASAP (must be signed by a parent /guardian to have a physical done at school). If your physical has been done please turn this in to the school nurse. Just a reminder, the 11th grade physical is a Pennsylvania State Law. Students will NOT be permitted to attend school at the beginning of the new school year until their physical form is turned in to the health office.
